IMHA Treatment Protocol in Dogs
Canine IMHA treatment is anchored in two consensus documents: the ACVIM consensus statement on the treatment of immune-mediated hemolytic anemia in dogs (Swann et al., J Vet Intern Med, 2019), which makes evidence-based clinical recommendations on immunosuppressive therapy (its indexed drug set is prednisolone, azathioprine, cyclosporine, and mycophenolate mofetil) and on antithrombotic therapy, and the ACVECC CURATIVE guidelines (Goggs et al., J Vet Emerg Crit Care, 2019), which supply the antithrombotic framework: 59 statements and 83 guideline recommendations, refined over 3 Delphi rounds from more than 500 reviewed studies, spanning populations at risk, rational therapeutic use, evidence-based protocols, monitoring, and discontinuation. The ACVIM panel derived its recommendations by extracting the published veterinary treatment literature, grading it with a purpose-built evidence-quality tool, benchmarking against human autoimmune hemolytic anemia guidelines, and refining through several iterations of Delphi review. The protocol is explicitly the regimen “to employ and maintain after diagnosis of the disease,” and the panel directs that it “should be implemented pragmatically, with consideration of animal, owner, and veterinary factors that may vary among cases.”
Immunosuppressive therapy
IMHA causes severe anemia in dogs and, per the consensus panel, “is associated with considerable morbidity and mortality”; the statement exists precisely because “little consensus exists among veterinarians as to the optimal regimen to employ and maintain after diagnosis of the disease.”
How the recommendations were built:
- Data were identified and extracted from studies published in the veterinary literature.
- A novel evidence-quality evaluation tool was developed and applied to study design, diagnostic criteria, explanation of treatment regimens, and validity of statistical methods.
- The panel combined its clinical experience with comparable guidelines for humans with autoimmune hemolytic anemia.
- Recommendations were refined through several iterations of Delphi review, with comments solicited from several professional bodies before publication.
- Emerging treatments were considered, and areas deserving of future research were highlighted.

Initial immunosuppression is prednisolone or prednisone at 2-3 mg/kg/day PO, or 50-60 mg/m2/day for dogs over 25 kg, given as a single or divided daily dose; IV dexamethasone at 0.2-0.4 mg/kg/day is acceptable temporarily if PO dosing is not tolerated (recommendation 7). If the starting dose is above 2 mg/kg/day, decrease to 2 mg/kg/day or less within the first 1-2 weeks in a dog that is responding, meaning a stable or increasing PCV/Hct (recommendation 8).
A second immunosuppressive drug may be added from the outset. The panel suggests two drugs when the presentation is severe or immediately life-threatening, when PCV/Hct drops 5% or more within 24 hours during the first 7 days, when the dog remains transfusion-dependent after 7 days, or when glucocorticoid adverse effects are expected, especially in dogs over 25 kg (recommendation 9). The second-agent doses (recommendation 10):
- Azathioprine: 2 mg/kg or 50 mg/m2 PO q24h; the interval may extend to every other day after 2-3 weeks.
- Cyclosporine: 5 mg/kg PO q12h, adjusted by therapeutic drug monitoring.
- Mycophenolate mofetil: 8-12 mg/kg PO q12h.
Monitoring for the second agents: azathioprine, CBC and ALT every 2 weeks for the first 2 months, then every 1-2 months; cyclosporine, biochemistry every 2-3 months; mycophenolate, CBC monitoring.
Transfusion support
The consensus statement is MeSH-indexed for Blood Transfusion (veterinary), placing transfusion support within its full-text scope alongside immunosuppressive and antithrombotic therapy.
The panel’s transfusion guidance is individualized rather than trigger-based: the decision to transfuse weighs severity of signs, resting lactate, the breed-normal PCV, speed of onset, and the monitoring available. Packed red blood cells are preferred over whole blood because dogs with IMHA are typically euvolemic, and routine plasma is not supported. Only 2 of the 8 panel members used a numerical trigger at all (PCV/Hct under 12% in an asymptomatic dog); the rest transfused on clinical grounds. There is no single consensus trigger number.
Antithrombotic therapy
The antithrombotic arm of the protocol is framed by the ACVECC CURATIVE guidelines, built as a systematic consensus in five domains: defining populations at risk; defining rational therapeutic use; defining evidence-based protocols; refining and monitoring antithrombotic therapies; and discontinuing antithrombotic therapies. Evidence evaluation used PICO-generated questions within each domain, with articles categorized by level of evidence and quality (Good, Fair, or Poor); consensus was achieved via Delphi-style surveys, and draft recommendations were presented at 2 international veterinary conferences and opened to community assessment, review, and comment before final revision.
The yield: over 500 studies reviewed in detail, 59 statements with 83 guideline recommendations refined during 3 rounds of Delphi surveys, and “a high degree of consensus … reached across all guideline recommendations.” The evaluations yielded more than 80 recommendations for small animals with or at risk of developing thrombosis. The ACVIM treatment consensus cites CURATIVE directly and is itself indexed on anticoagulants and platelet aggregation inhibitors (administration & dosage, therapeutic use). The CURATIVE panel’s own conclusion flags the evidence base’s limits: “Numerous significant knowledge gaps were highlighted by the evidence reviews undertaken, indicating the need for substantial additional research in this field.”
The IMHA-specific antithrombotic recommendations come from the ACVIM consensus itself, applying that framework to this disease. The panel recommends thromboprophylaxis for all dogs with IMHA except those with severe thrombocytopenia (platelet count under 30,000/uL), starting at diagnosis and continuing until remission and off glucocorticoids; it may stop after 6 months of remission if there are no other risk factors (recommendations 38 and 39). A regimen incorporating anticoagulants may be preferred, especially during the first 2 weeks; if anticoagulants with monitoring are not feasible, antiplatelet drugs are preferred over nothing (recommendation 40). Among anticoagulants, individually anti-Xa-adjusted unfractionated heparin (UFH) is preferred, with these suggested starting doses (recommendation 41):
- UFH IV: 100 U/kg bolus, then 900 U/kg/24 h.
- UFH SC: 150-300 U/kg q6h.
- Dalteparin SC: 150-175 U/kg q8h.
- Enoxaparin SC: 0.8-1.0 mg/kg q6-8h.
- Rivaroxaban PO: 1-2 mg/kg q24h.
Among antiplatelet drugs, clopidogrel is preferred over aspirin, dosed at 1.1-4.0 mg/kg PO q24h with an optional single loading dose (double the maintenance dose, up to 10 mg/kg); if aspirin is used, it is dosed at 1-2 mg/kg q24h and may be combined with clopidogrel (recommendation 42).
Maintenance, monitoring, and relapse
Both documents treat IMHA therapy as an ongoing protocol rather than a one-time induction decision. The ACVIM consensus is framed around the regimen “to employ and maintain after diagnosis of the disease,” and two of CURATIVE’s five domains address continuing care directly: refining and monitoring antithrombotic therapies, and discontinuing antithrombotic therapies.
The consensus sets the glucocorticoid taper explicitly: decrease prednisone or prednisolone by 25% once the PCV/Hct is stable and above 30% for 2 weeks with improving disease activity, then continue decreasing by 25% every 3 weeks; with a second agent in place, taper steps of 25-33% or 2-week intervals are possible (recommendations 14 and 15).
Clinical context and limitations
These are 2019 consensus and guideline documents that grade the veterinary evidence available at the time, synthesizing existing studies rather than reporting new trials, and the CURATIVE panel explicitly acknowledged substantial knowledge gaps in the antithrombotic evidence. The ACVIM statement addresses dogs specifically; CURATIVE covers small animals (dogs and cats) generally, so its recommendations require species-specific application.
